Pypesmusic - Request Form

STATEMENT OF AGREEMENT

Any request made in less than 10 days of an event, will be considered a late request. If a late request is excepted by pypesmusic, a late request fee of $100.00 will be added to the cost of the performance. All travel cost are paid by the requester if not drivable. All drivable distances are paid at 55 cents per mile to and from performer's residence/place of business. A driving log must be provided by performer for payment.
